Adopter Change of Information Form
As part of your contract, you have agreed to contact Serendipity German Shepherd Dog Rescue to notify us if your current address or phone number changes. Please do take the time to call, email or return the following form in the mail to us within 30 days of your address change, and include any other change as well – such as veterinary care provider.
If your address hasn't changed, but you want to send us an update about a dog you adopted from us, please visit our Happy Tails update page.
